Summary
Procainamide and its active metabolite are cationic drugs eliminated partly by renal tubular secretion, and metformin uses the same pathway. Concurrent use may raise exposure to either drug, increasing the risk of procainamide toxicity such as arrhythmia or QRS/QT prolongation, and of metformin accumulation.
What this interaction means in plain words
Dapagliflozin-metformin and Procainamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Competition for renal tubular cationic transporters (OCT2/MATE) reduces the clearance of both drugs.
What to do
Monitor ECG and procainamide plasma concentrations and watch for signs of metformin excess, including GI upset and lactic acidosis. Reduce the dose of either drug if toxicity appears, especially in renal impairment.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
