Summary

ACE inhibitors may enhance insulin sensitivity and occasionally cause hypoglycemia in patients on antidiabetic drugs. Also, ACE inhibitors can reduce renal function in susceptible patients, which may lead to metformin accumulation and lactic acidosis, and can worsen volume depletion with dapagliflozin.

What this interaction means in plain words

Dapagliflozin-metformin and Quinapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

ACE inhibition may improve insulin sensitivity and can reduce renal perfusion, decreasing metformin clearance; the evidence is mixed.

What to do

Monitor blood glucose and renal function (eGFR) after starting or adjusting quinapril. Hold metformin if renal function deteriorates significantly or volume depletion occurs.

What to ask your pharmacist
