Summary
Topiramate inhibits carbonic anhydrase and can cause hyperchloraemic non-anion gap metabolic acidosis. Combined with metformin, which carries a lactic acidosis risk, the acidosis may be worsened or harder to interpret. Topiramate may also modestly raise metformin exposure.
What this interaction means in plain words
Dapagliflozin-metformin and Topiramate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dapagliflozin-metformin belongs to Endocrine; Topiramate belongs to Central Nervous System.
Why it happens
Carbonic anhydrase inhibition lowers serum bicarbonate, and the resulting acidosis adds to metformin-associated lactic acidosis risk.
What to do
Measure serum bicarbonate at baseline and periodically. Consider stopping or reducing metformin or topiramate if persistent metabolic acidosis develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
