For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Deferasirox can induce CYP3A4 modestly and may lower propafenone concentrations, though propafenone is cleared mainly by CYP2D6 with CYP3A4 and CYP1A2 as secondary routes. Reduced antiarrhythmic effect is possible. Deferasirox also inhibits CYP2D6 weakly, which could have the opposite effect.

What this interaction means in plain words

Deferasirox and Propafenone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Deferasirox belongs to Miscellaneous; Propafenone belongs to Cardiovascular.

Evidence level: Low

Why it happens

Deferasirox alters CYP enzyme activity (CYP3A4 induction, CYP2D6 and CYP1A2 inhibition), changing propafenone exposure unpredictably.

What to do

Monitor heart rhythm and clinical response to propafenone when starting or stopping deferasirox. Check ECG and adjust dose as needed.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
