Summary

Nilotinib carries a boxed warning for QT prolongation and sudden death, and propafenone adds proarrhythmic effects. Nilotinib also inhibits CYP3A4 and CYP2D6, which may increase propafenone concentrations and further raise the risk of arrhythmia.

What this interaction means in plain words

Nilotinib and Propafenone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Nilotinib belongs to Oncology; Propafenone belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive QT prolongation combined with nilotinib inhibition of CYP3A4 and CYP2D6, increasing propafenone exposure.

What to do

Avoid the combination and choose a non-QT-prolonging alternative for rhythm control. If nilotinib cannot be replaced, switch propafenone to an agent without QT or CYP interactions.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
