Summary

Selexipag is converted to its active metabolite partly through CYP2C8, and deferasirox inhibits CYP2C8 moderately. Exposure to the active metabolite may increase, leading to more prostacyclin-related adverse effects such as headache, diarrhea and jaw pain.

What this interaction means in plain words

Deferasirox and Selexipag can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Deferasirox belongs to Miscellaneous; Selexipag belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Deferasirox inhibits CYP2C8, reducing metabolism of selexipag and its active metabolite.

What to do

Reduce selexipag to once-daily dosing while deferasirox is used. Watch for adverse effects and return to the usual schedule if deferasirox is stopped.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
