Summary

Corticosteroids are lymphotoxic and may blunt the expansion and persistence of CAR-T cells, which could reduce the efficacy of tisagenlecleucel. They also add to infection risk in an already immunocompromised patient. Steroids are still used therapeutically for cytokine release syndrome or neurotoxicity.

What this interaction means in plain words

Deflazacort and Tisagenlecleucel can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Deflazacort belongs to Miscellaneous; Tisagenlecleucel belongs to Oncology.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism: glucocorticoid-mediated suppression and depletion of T lymphocytes reduces CAR-T cell activity, with additive immunosuppression.

What to do

Avoid prophylactic or premedication use of deflazacort around CAR-T infusion unless clinically essential. Reserve corticosteroids for managing cytokine release syndrome or neurologic toxicity, and monitor for infection.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
