Summary

Both isotretinoin and tetracyclines such as demeclocycline can independently cause raised intracranial pressure. Combined use increases the risk of pseudotumor cerebri, with headache, visual disturbance and papilledema, and potential permanent vision loss.

What this interaction means in plain words

Demeclocycline and Isotretinoin are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic effect on intracranial pressure; exact mechanism not established.

What to do

Avoid concurrent use; choose a non-tetracycline antibiotic. If headache or visual symptoms develop, stop both and obtain urgent ophthalmologic evaluation.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
