Summary
Tropisetron, a 5-HT3 antagonist, may prolong the QT interval, and desipramine can do likewise. Concurrent use may increase the risk of arrhythmia, especially when other risk factors are present.
What this interaction means in plain words
Desipramine and Tropisetron can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive prolongation of cardiac repolarisation. Tropisetron is also metabolised by CYP2D6, which desipramine can inhibit weakly.
What to do
Obtain an ECG in patients with cardiac disease or electrolyte abnormalities and correct potassium and magnesium. Consider an alternative antiemetic if QTc is prolonged.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
