Summary
Fezolinetant is metabolized mainly by CYP1A2, and ethinyl estradiol is a weak CYP1A2 inhibitor that can raise fezolinetant exposure. Fezolinetant labeling contraindicates use with CYP1A2 inhibitors, and its indication for menopausal vasomotor symptoms means combined contraceptive use is rarely appropriate anyway. Increased exposure raises the risk of hepatotoxicity.
What this interaction means in plain words
Desogestrel-ethinyl estradiol and Fezolinetant are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Desogestrel-ethinyl estradiol belongs to Endocrine; Fezolinetant belongs to Endocrine.
Why it happens
Ethinyl estradiol inhibits CYP1A2, reducing fezolinetant metabolism and increasing its plasma concentration.
What to do
Avoid coadministration. Use an alternative approach for vasomotor symptoms or contraception, and monitor liver tests if fezolinetant is used.
What to ask your pharmacist
