Dexamethasone causes insulin resistance and hyperglycemia, opposing the effect of metformin and pioglitazone. Dexamethasone may also modestly induce CYP3A4, which could lower pioglitazone exposure. Glucose control can deteriorate noticeably, particularly with higher doses or prolonged courses.
What this interaction means in plain words
Dexamethasone and Metformin-pioglitazone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dexamethasone belongs to Endocrine; Metformin-pioglitazone belongs to Endocrine.
Why it happens
Glucocorticoid-induced hepatic gluconeogenesis and peripheral insulin resistance oppose antidiabetic action, with possible mild CYP3A4 induction.
What to do
Monitor blood glucose more frequently during corticosteroid therapy and increase antidiabetic doses as needed; reassess doses when the steroid is tapered to avoid hypoglycemia.
