Summary
Repaglinide is metabolized by CYP2C8 and CYP3A4, and dexamethasone may induce CYP3A4, reducing repaglinide exposure. Dexamethasone also raises blood glucose directly. Both effects can reduce glycemic control.
What this interaction means in plain words
Dexamethasone and Repaglinide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dexamethasone belongs to Endocrine; Repaglinide belongs to Endocrine.
Why it happens
Dexamethasone induces CYP3A4 and causes steroid-induced hyperglycemia, opposing repaglinide effect.
What to do
Monitor blood glucose frequently during and after steroid therapy and adjust repaglinide dose accordingly. Watch for hypoglycemia when the steroid is tapered.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
