Summary
Lorlatinib is a CYP3A4 inducer and may reduce dexamethasone exposure. In addition, dexamethasone, as a CYP3A4 inducer, may lower lorlatinib levels, and concomitant strong inducers are restricted in labeling because of reduced efficacy and hepatotoxicity concerns.
What this interaction means in plain words
Dexamethasone-neomycin-polymyxin and Lorlatinib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dexamethasone-neomycin-polymyxin belongs to Anti Bacterial; Lorlatinib belongs to Oncology.
Why it happens
Mutual CYP3A4 induction accelerates metabolism of dexamethasone and lorlatinib.
What to do
Monitor clinical response to dexamethasone and consider dose adjustment. Monitor liver enzymes and lorlatinib effectiveness if systemic dexamethasone is used long term.
