Summary
Metyrapone blocks cortisol synthesis and is used diagnostically by measuring the rise in ACTH and 11-deoxycortisol. Dexamethasone suppresses the pituitary-adrenal axis, so it can blunt the response and make the test unreliable. Topical ophthalmic use would have far less effect than systemic use.
What this interaction means in plain words
Dexamethasone-tobramycin and Metyrapone have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Dexamethasone-tobramycin belongs to Anti Bacterial; Metyrapone belongs to Endocrine.
Why it happens
Exogenous corticosteroid suppresses ACTH release through negative feedback, opposing the compensatory ACTH rise that metyrapone relies on.
What to do
Avoid systemic dexamethasone before and during metyrapone testing, or interpret results with the steroid exposure in mind. Use an alternative assessment method if corticosteroid therapy cannot be stopped.
