Summary

Voclosporin is a calcineurin inhibitor that can cause nephrotoxicity, and tobramycin can also injure the kidney when absorbed systemically. Together they may increase the risk of kidney injury. Ophthalmic tobramycin has little systemic exposure, reducing this risk.

What this interaction means in plain words

Dexamethasone-tobramycin and Voclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dexamethasone-tobramycin belongs to Anti Bacterial; Voclosporin belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Additive nephrotoxicity from calcineurin inhibitor-induced renal vasoconstriction and aminoglycoside tubular toxicity.

What to do

If systemic tobramycin is needed, monitor serum creatinine and eGFR and tobramycin levels closely. Adjust the dose of either drug if renal function deteriorates.

What to ask your pharmacist
