Summary
Diclofenac can blunt the blood pressure lowering effect of olmesartan. In patients who are elderly, volume depleted or have existing renal impairment, the combination can precipitate acute kidney injury and raise potassium.
What this interaction means in plain words
Diclofenac and Olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Diclofenac belongs to Musculoskeletal; Olmesartan belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, opposing angiotensin receptor blockade and impairing glomerular filtration.
What to do
Monitor blood pressure, serum creatinine and potassium after starting or changing the NSAID, and ensure adequate hydration. Use the lowest effective diclofenac dose for the shortest time, or choose an alternative analgesic in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
