Why it happens

NSAIDs inhibit renal prostaglandin synthesis, reducing afferent arteriolar dilation and sodium excretion, which opposes ARB effect and reduces glomerular filtration.

What to do

Use the lowest effective NSAID dose for the shortest time, ensure hydration, and monitor blood pressure, creatinine and potassium. Consider paracetamol or a topical agent instead.
