Summary

Both agents are neurotoxic, and co-administration can increase the risk and severity of peripheral neuropathy. Vincristine neuropathy is already dose-limiting, and didanosine adds to it.

What this interaction means in plain words

Didanosine and Vincristine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Didanosine belongs to Anti Virals; Vincristine belongs to Oncology.

Evidence level: Moderate

Why it happens

Additive peripheral neurotoxicity through different mechanisms (mitochondrial toxicity with didanosine, microtubule disruption with vincristine).

What to do

Prefer an alternative antiretroviral to didanosine during vincristine therapy. If unavoidable, assess neurological status before each vincristine dose and reduce or stop drugs at early neuropathy signs.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
