Summary
Oral tranexamic acid is antifibrinolytic and estrogen-containing contraceptives are prothrombotic. Together they raise the risk of venous and arterial thrombosis, and oral tranexamic acid labeling contraindicates use with combined hormonal contraceptives.
What this interaction means in plain words
Dienogest / estradiol valerate and Tranexamic acid are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Additive prothrombotic effects of fibrinolysis inhibition and estrogen-induced increases in clotting factors.
What to do
Avoid the combination; use non-hormonal or progestin-only contraception while taking tranexamic acid, or choose another heavy-bleeding treatment.
