Summary

Oral tranexamic acid is antifibrinolytic and ethinylestradiol is prothrombotic. Together they raise the risk of venous and arterial thrombosis, and oral tranexamic acid labeling contraindicates use with combined hormonal contraceptives.

What this interaction means in plain words

Ethinylestradiol and Tranexamic acid are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Additive prothrombotic effects of fibrinolysis inhibition and estrogen-induced increases in clotting factors.

What to do

Avoid the combination; use non-hormonal or progestin-only contraception while taking tranexamic acid, or choose another heavy-bleeding treatment.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
