Summary
Diflunisal impairs platelet function and can injure the gastric mucosa, so combining it with heparin increases the risk of clinically significant bleeding. The risk is highest with prolonged use or therapeutic heparin doses.
What this interaction means in plain words
Diflunisal and Heparin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Diflunisal belongs to Musculoskeletal; Heparin belongs to Cardiovascular.
Why it happens
Additive hemostatic impairment: NSAID platelet inhibition and mucosal injury plus heparin-mediated anticoagulation.
What to do
Avoid concurrent use where possible and prefer paracetamol for analgesia. If combined, monitor for bleeding, hemoglobin and aPTT or anti-Xa as appropriate.
