Summary
NSAIDs can reduce the antihypertensive effect of angiotensin receptor blockers and, together, may impair kidney function, especially in elderly, dehydrated or renally impaired patients. Hyperkalemia may also occur.
What this interaction means in plain words
Diflunisal and Olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Diflunisal belongs to Musculoskeletal; Olmesartan belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal prostaglandins reduces afferent arteriolar dilation while the ARB dilates the efferent arteriole, lowering glomerular filtration and promoting sodium retention and potassium retention.
What to do
Monitor blood pressure, serum creatinine and potassium within the first weeks and periodically thereafter. Ensure hydration and avoid prolonged NSAID use in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
