Summary
Diflunisal can reduce the antihypertensive effect of quinapril and increase the risk of acute kidney injury, particularly in elderly, dehydrated or renally impaired patients. Hyperkalemia may also be more likely.
What this interaction means in plain words
Diflunisal and Quinapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal blood flow and sodium excretion, counteracting ACE inhibition.
What to do
Use the lowest effective NSAID dose for the shortest time and ensure adequate hydration. Monitor blood pressure, serum creatinine and potassium, particularly at the start of therapy.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
