Summary
NSAIDs can blunt the blood pressure lowering effect of angiotensin receptor blockers. In volume-depleted or elderly patients, the combination may cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Diflunisal and Telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Diflunisal belongs to Musculoskeletal; Telmisartan belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal vasodilatory prostaglandins reduces glomerular perfusion while ARB blocks efferent arteriolar tone, impairing filtration and potassium excretion.
What to do
Ensure adequate hydration and monitor blood pressure, serum creatinine and potassium after starting or changing the NSAID. Use the lowest effective NSAID dose for the shortest time.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
