Summary

Both drugs increase noradrenergic activity, so together they can cause excessive blood pressure elevation, including supine hypertension. This is clinically important because droxidopa is used in patients with neurogenic orthostatic hypotension who already risk supine hypertension.

What this interaction means in plain words

Droxidopa and Pseudoephedrine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Droxidopa belongs to Cardiovascular; Pseudoephedrine belongs to Respiratory.

Evidence level: Moderate

Why it happens

Additive pressor effects: droxidopa is converted to norepinephrine while pseudoephedrine releases norepinephrine.

What to do

Avoid the combination if possible; if used, monitor supine and standing blood pressure and keep the head of the bed elevated. Reduce or stop one agent if blood pressure rises excessively.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
