Summary
Echothiophate is an irreversible cholinesterase inhibitor and donepezil is a reversible one, so combining them stacks cholinergic stimulation. This can cause bradycardia, bronchospasm, excess secretions, gastrointestinal cramping and muscle weakness. Systemic absorption of ophthalmic echothiophate is limited but not zero.
What this interaction means in plain words
Echothiophate iodide and Memantine-donepezil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive acetylcholinesterase inhibition produces summed cholinergic (muscarinic and nicotinic) effects.
What to do
Monitor heart rate and for GI, respiratory and neuromuscular cholinergic signs when both are used. Reduce or stop one agent if symptoms of cholinergic excess appear.
What to ask your pharmacist
