Summary

Topical econazole applied over large areas, to genital skin or under occlusive dressings can be absorbed enough to raise the INR in patients taking warfarin. Bleeding episodes have been reported with azole antifungals used this way.

What this interaction means in plain words

Econazole and Warfarin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Econazole belongs to Anti Fungals; Warfarin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Systemically absorbed econazole inhibits CYP2C9 and other CYP enzymes, reducing clearance of the more potent S-warfarin.

What to do

Check INR within several days of starting econazole, particularly with extensive, genital or occluded application, and adjust the warfarin dose as needed. Limit the treated area where possible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
