Summary
Intravenous vancomycin and the tenofovir disoproxil-containing combination can each injure the kidney. Used together they raise the likelihood of acute kidney injury, and cobicistat may further increase serum creatinine by inhibiting tubular secretion.
What this interaction means in plain words
Elvitegravir / cobicistat / emtricitabine / tenofovir df and Vancomycin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive nephrotoxicity from vancomycin tubular injury and tenofovir proximal tubule toxicity.
What to do
Monitor serum creatinine, estimated creatinine clearance and urine protein or glucose at baseline and regularly during the vancomycin course. Check vancomycin levels and stop the regimen if renal function declines significantly.
