Summary
ACE inhibitors can modestly improve insulin sensitivity and occasionally cause hypoglycemia in patients on antidiabetic drugs. They can also reduce renal function in volume-depleted patients, which, together with the diuretic effect of empagliflozin, may raise metformin exposure and the risk of lactic acidosis.
What this interaction means in plain words
Empagliflozin-metformin and Lisinopril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Empagliflozin-metformin belongs to Endocrine; Lisinopril belongs to Cardiovascular.
Why it happens
Additive blood pressure and volume effects with possible renal function decline that reduces metformin clearance; ACE inhibitors may also enhance insulin sensitivity.
What to do
Monitor renal function, potassium, volume status, and glucose after starting or adjusting therapy. Hold metformin if acute kidney injury or dehydration occurs.
What to ask your pharmacist
