Summary
Opioids may cause hypotension and can induce antidiuretic hormone release, which may blunt diuretic effect and cause fluid retention. Together with an ACE inhibitor and thiazide, hypotension may be more pronounced, particularly during anesthesia or after dose increases.
What this interaction means in plain words
Enalapril-hydrochlorothiazide and Fentanyl can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Enalapril-hydrochlorothiazide belongs to Cardiovascular; Fentanyl belongs to Musculoskeletal.
Why it happens
Opioid-induced histamine release and vasodilation add to antihypertensive effect, while ADH release may counteract diuresis.
What to do
Monitor blood pressure, fluid status and urine output during fentanyl use. Adjust the antihypertensive or diuretic dose if hypotension or fluid retention occurs.
