Summary
Ganciclovir is renally eliminated, partly by active tubular secretion. Hydrochlorothiazide may slightly reduce its clearance, and diuretic-induced volume depletion or reduced renal function could further increase exposure. Higher ganciclovir levels raise the risk of myelosuppression and other toxicity.
What this interaction means in plain words
Enalapril-hydrochlorothiazide and Ganciclovir have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Enalapril-hydrochlorothiazide belongs to Cardiovascular; Ganciclovir belongs to Anti Virals.
Why it happens
Possible competition for renal tubular secretion and reduced renal perfusion, decreasing ganciclovir clearance.
What to do
Dose ganciclovir according to creatinine clearance and monitor renal function and blood counts during therapy.
