Summary

Heparin suppresses adrenal aldosterone synthesis, which can cause hyperkalemia. Combined with an ACE inhibitor, which also lowers aldosterone, the risk of raised potassium increases, particularly in patients with renal impairment or diabetes.

What this interaction means in plain words

Enalapril-hydrochlorothiazide and Heparin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Enalapril-hydrochlorothiazide belongs to Cardiovascular; Heparin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin inhibits aldosterone secretion, adding to ACE inhibitor-mediated reduction of aldosterone and potassium excretion.

What to do

Monitor serum potassium and renal function, especially with prolonged heparin therapy or in high-risk patients. Treat hyperkalemia or review therapy if potassium rises.

What to ask your pharmacist
