Summary
Thiazide diuretics may reduce renal elimination of methotrexate and have been reported to prolong and intensify myelosuppression. ACE inhibitors and diuretics can also reduce renal perfusion, which further impairs methotrexate clearance. This matters most with high-dose methotrexate or reduced renal function.
What this interaction means in plain words
Enalapril-hydrochlorothiazide and Methotrexate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Enalapril-hydrochlorothiazide belongs to Cardiovascular; Methotrexate belongs to Oncology.
Why it happens
Reduced renal clearance of methotrexate via decreased tubular secretion and diuretic or ACE inhibitor effects on renal hemodynamics.
What to do
Monitor blood counts and renal function closely, and consider methotrexate dose reduction or an alternative antihypertensive, especially with high-dose methotrexate.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
