Summary
Noni juice has a high potassium content and can raise serum potassium. Enalapril also retains potassium, so hyperkalemia is possible, especially in patients with kidney impairment, although the thiazide partially offsets this.
What this interaction means in plain words
Enalapril-hydrochlorothiazide and Noni juice can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Dietary potassium load combines with ACE inhibitor-induced reduction in aldosterone-mediated potassium excretion, partly countered by thiazide kaliuresis.
What to do
Advise avoiding regular noni juice intake, particularly with renal impairment, and check serum potassium if it is consumed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
