Summary

Enalapril reduces aldosterone and thus renal potassium excretion, so potassium acid phosphate supplements can cause hyperkalemia. The thiazide partly offsets this, but the risk remains, especially with renal impairment or diabetes.

What this interaction means in plain words

Enalapril-hydrochlorothiazide and Potassium acid can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: High

Why it happens

ACE inhibitor-induced reduction in aldosterone and renal potassium excretion adds to an exogenous potassium load.

What to do

Check serum potassium and renal function before and shortly after starting supplementation, then periodically. Reduce or stop the supplement if potassium rises.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
