Summary
Trilostane reduces adrenal steroid synthesis including aldosterone, which lessens potassium loss from thiazides. Combined with enalapril, which also lowers aldosterone, there is a risk of hyperkalemia and hypotension.
What this interaction means in plain words
Enalapril-hydrochlorothiazide and Trilostane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of 3-beta-hydroxysteroid dehydrogenase lowers aldosterone, adding to ACE inhibitor potassium retention.
What to do
Monitor serum potassium, sodium and blood pressure when combined.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
