Summary
Vilanterol, a beta-agonist, can lower serum potassium and may cause ECG changes, and hydrochlorothiazide adds to this effect. The risk is higher with excessive beta-agonist doses.
What this interaction means in plain words
Enalapril-hydrochlorothiazide and Umeclidinium bromide / vilanterol inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive hypokalemia from beta-2 stimulated intracellular potassium shift and thiazide renal potassium loss; the ACE inhibitor partly offsets it.
What to do
Monitor serum potassium in patients at risk, such as those with cardiac disease or on high doses. Do not exceed the recommended inhaled dose.
What to ask your pharmacist
