Summary

Encorafenib can both inhibit and induce CYP3A4 and also inhibits hepatic uptake and efflux transporters such as OATP1B1, OATP1B3 and BCRP. Simvastatin exposure may therefore rise, increasing the risk of myopathy, or fall, reducing lipid-lowering effect, depending on which effect predominates.

What this interaction means in plain words

Encorafenib and Simvastatin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Encorafenib belongs to Oncology; Simvastatin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Mixed CYP3A4 inhibition and induction plus inhibition of OATP1B1/1B3 and BCRP transporters by encorafenib alters simvastatin disposition.

What to do

Monitor for muscle pain, weakness and CK elevation, and consider using the lowest effective simvastatin dose or switching to a statin less dependent on CYP3A4. Check lipid response during prolonged co-therapy.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
