Summary

Verapamil is a moderate CYP3A4 inhibitor and can raise encorafenib exposure. Higher encorafenib levels may increase adverse effects such as QT prolongation and dermatologic or other toxicities.

What this interaction means in plain words

Encorafenib and Verapamil can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Encorafenib belongs to Oncology; Verapamil belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Verapamil inhibits CYP3A4-mediated metabolism of encorafenib, a CYP3A4 substrate.

What to do

Prefer an alternative to verapamil. If unavoidable, reduce the encorafenib dose to about half, monitor QTc and toxicity, and restore the usual dose 3-5 half-lives after stopping verapamil.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
