Summary

Verapamil is a moderate CYP3A4 inhibitor and can raise lomitapide exposure several-fold. Higher lomitapide levels increase the risk of hepatotoxicity and gastrointestinal adverse effects. Labeling for lomitapide prohibits use with moderate and strong CYP3A4 inhibitors.

What this interaction means in plain words

Lomitapide and Verapamil are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Lomitapide belongs to Cardiovascular; Verapamil belongs to Cardiovascular.

Evidence level: High

Why it happens

Verapamil inhibits CYP3A4-mediated first-pass and hepatic metabolism of lomitapide, markedly increasing its plasma concentration.

What to do

Do not co-administer; choose a different antihypertensive or antianginal such as a non-CYP3A4-inhibiting agent. Monitor liver enzymes if lomitapide therapy is continued with an alternative.

What to ask your pharmacist
