Summary
Low molecular weight heparins can suppress aldosterone secretion and thereby raise serum potassium. With ramipril, which also lowers aldosterone, hyperkalemia risk is increased, mainly in patients with renal impairment, diabetes or prolonged use.
What this interaction means in plain words
Enoxaparin and Ramipril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Enoxaparin belongs to Cardiovascular; Ramipril belongs to Cardiovascular.
Why it happens
Heparin-induced suppression of adrenal aldosterone synthesis adds to ACE inhibitor-mediated reduction of potassium excretion.
What to do
Monitor serum potassium and renal function, particularly if enoxaparin is given for more than a few days or in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
