Summary

Heparins can suppress adrenal aldosterone secretion, which may cause hyperkalemia. Together with an ACE inhibitor the risk is additive, especially in diabetes, renal impairment or prolonged use.

What this interaction means in plain words

Enoxaparin and Trandolapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Enoxaparin belongs to Cardiovascular; Trandolapril belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin-induced inhibition of aldosterone synthesis added to ACE inhibitor reduction of angiotensin II-driven aldosterone release.

What to do

Monitor serum potassium, particularly if enoxaparin is given for more than a few days or renal function is reduced. Treat hyperkalemia if it develops.
