Summary

Epinephrine and salbutamol both stimulate beta-adrenergic receptors, so together they can cause marked tachycardia, arrhythmias, tremor, hypertension and hypokalemia. The risk is higher with systemic epinephrine and in patients with cardiac disease.

What this interaction means in plain words

Epinephrine and Ipratropium-salbutamol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Epinephrine belongs to Endocrine; Ipratropium-salbutamol belongs to Respiratory.

Evidence level: Moderate

Why it happens

Additive adrenergic receptor stimulation and beta-2 mediated intracellular potassium shift.

What to do

Monitor heart rate, rhythm, blood pressure and serum potassium when both are used. Avoid giving additional salbutamol doses unnecessarily around epinephrine administration.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
