Summary
Quetiapine blocks alpha-1 adrenergic receptors, which can blunt the pressor effect of epinephrine and leave beta-mediated vasodilation unopposed, resulting in hypotension and reflex tachycardia. This is most relevant when epinephrine is given systemically for anaphylaxis or shock.
What this interaction means in plain words
Epinephrine and Quetiapine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Epinephrine belongs to Endocrine; Quetiapine belongs to Central Nervous System.
Why it happens
Alpha-1 adrenergic blockade by quetiapine opposes epinephrine vasoconstriction, leaving beta-2 vasodilation unopposed.
What to do
Monitor blood pressure and heart rate closely when systemic epinephrine is given; if vasopressor support is needed and hypotension persists, consider norepinephrine or phenylephrine. Epinephrine should still not be withheld in anaphylaxis.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

How serious is the interaction between Epinephrine and Quetiapine?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor blood pressure and heart rate closely when systemic epinephrine is given; if vasopressor support is needed and hypotension persists, consider norepinephrine or phenylephrine. Epinephrine should still not be withheld in anaphylaxis.
