Summary
Epinephrine can provoke arrhythmias and ventricular repolarization changes, and ziprasidone prolongs the QT interval. Ziprasidone's alpha-blocking activity may also cause unopposed beta-mediated vasodilation and hypotension with epinephrine. The overall risk depends on dose and route.
What this interaction means in plain words
Epinephrine and Ziprasidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Epinephrine belongs to Endocrine; Ziprasidone belongs to Central Nervous System.
Why it happens
Additive proarrhythmic effects, plus alpha-adrenergic blockade by ziprasidone altering the vascular response to epinephrine.
What to do
Do not withhold epinephrine in anaphylaxis or cardiac arrest. For non-emergency use such as local anesthetic mixtures, use the lowest effective dose and monitor ECG and blood pressure.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
