Summary
Nonselective MAO inhibition can intensify the cardiovascular effects of racemic epinephrine. Severe hypertension, tachyarrhythmia and hypertensive crisis may result.
What this interaction means in plain words
Epinephrine racemic and Tranylcypromine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
MAO inhibition increases stores of norepinephrine and reduces catecholamine metabolism, resulting in additive adrenergic effects.
What to do
Avoid combined use and use non-adrenergic alternatives such as saline or corticosteroids for airway swelling where appropriate. In an emergency such as anaphylaxis, give epinephrine only with continuous blood pressure and ECG monitoring.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
