Summary

Eprosartan and valsartan are both angiotensin receptor blockers, so combining them duplicates therapy. This increases the risk of hypotension, hyperkalemia and renal impairment without added benefit.

What this interaction means in plain words

Eprosartan and Hydrochlorothiazide-valsartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Eprosartan belongs to Cardiovascular; Hydrochlorothiazide-valsartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Duplicate blockade of the angiotensin II type 1 receptor increases hypotensive and potassium-retaining effects.

What to do

Avoid using two ARBs together and use only one. If this combination was given, stop one agent and check blood pressure, creatinine and potassium.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
