Summary
NSAIDs can blunt the blood pressure lowering effect of angiotensin receptor blockers. In volume-depleted or elderly patients, or those with renal impairment, the combination can precipitate acute kidney injury and hyperkalemia.
What this interaction means in plain words
Eprosartan and Meloxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eprosartan belongs to Cardiovascular; Meloxicam belongs to Musculoskeletal.
Why it happens
Meloxicam inhibits vasodilatory renal prostaglandins, reducing afferent arteriolar flow while eprosartan blocks efferent tone, which lowers glomerular filtration and impairs potassium excretion.
What to do
Ensure adequate hydration and monitor blood pressure, serum creatinine and potassium after starting and periodically. Use the lowest effective meloxicam dose for the shortest time, or choose an alternative analgesic in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
