Summary
NSAIDs can blunt the antihypertensive effect of angiotensin receptor blockers. Together they can also impair renal function and raise potassium, especially in older, volume-depleted or renally impaired patients.
What this interaction means in plain words
Eprosartan and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eprosartan belongs to Cardiovascular; Tolmetin belongs to Musculoskeletal.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins reduces renal perfusion and opposes ARB effects, compounding the effect on glomerular filtration.
What to do
Monitor blood pressure, serum creatinine and potassium, especially at initiation, and ensure adequate hydration. Use the lowest NSAID dose for the shortest time or choose an alternative analgesic.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
