Summary
Ibuprofen can reduce the antihypertensive effect of the ARB and thiazide combination. Together with a diuretic and ARB it increases the risk of acute kidney injury and hyperkalemia, especially in older or dehydrated patients.
What this interaction means in plain words
Eprosartan-hydrochlorothiazide and Ibuprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eprosartan-hydrochlorothiazide belongs to Cardiovascular; Ibuprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandins causes sodium and water retention and lowers renal perfusion, opposing the diuretic and ARB and impairing kidney function.
What to do
Monitor blood pressure, creatinine and potassium, and maintain hydration. Prefer paracetamol or the lowest effective NSAID dose for a short course.
